Yumi Health is a privately operated insurance agency. We are not a government agency, nor are we affiliated with or endorsed by any federal or state government entity. Our website and services are not connected to the federal Health Insurance Marketplace or any state exchange.
CMS Required Medicare Disclosure (TPMO)
We do not offer every plan available in your area. Currently we represent multiple insurance organizations which offer products in your area. Please contact Medicare.gov, 1-800-MEDICARE (1-800-633-4227), or your local State Health Insurance Assistance Program (SHIP) to get information on all of your options. TTY users: 1-877-486-2048. Yumi Health is not affiliated with, endorsed by, or connected to the Centers for Medicare & Medicaid Services (CMS), the U.S. Department of Health and Human Services, or any other government agency. Medicare is a federal program administered by CMS. Our role is to help you understand and enroll in Medicare-related products through licensed agents appointed with individual carriers.
Licensed Insurance Agents
All insurance agents at Yumi Health are licensed in New Jersey and appointed with the insurance carriers whose products they offer. Agent licensing and carrier appointments are verified and monitored on an ongoing basis. Agents are bound by CMS marketing guidelines, NJ DOBI regulations, and all applicable state and federal laws governing the sale and marketing of insurance products. Agents must complete annual AHIP certification and carrier-specific certifications before marketing Medicare plans.
Scope of Appointment (SOA) — Medicare
In compliance with CMS regulations, a Scope of Appointment (SOA) form must be completed and signed prior to any Medicare sales appointment. The SOA documents which Medicare products the beneficiary has agreed to discuss. Our agents are required to obtain a completed SOA before any Medicare Advantage or Part D plan discussion. SOA forms are retained in our compliance records for a minimum of 10 years as required by CMS.
ACA / Marketplace Disclosure
Yumi Health is not affiliated with or endorsed by the federal Health Insurance Marketplace (HealthCare.gov) or any state marketplace exchange. ACA-qualified health plans are subject to open enrollment and special enrollment periods. Premium tax credits and cost-sharing reductions are determined by the federal government based on household income and other factors. Plan availability, premiums, and benefits vary by location and are subject to annual changes.
No Guarantee of Coverage
Yumi Health and its agents do not guarantee that any particular insurance plan will be available to you, that you will qualify for any specific coverage, or that any plan will cover specific treatments, procedures, or medications. All insurance products are subject to the terms, conditions, exclusions, and limitations set by the issuing carrier. Enrollment is subject to CMS enrollment windows, carrier underwriting where applicable, and eligibility requirements.
